- Line 1 : Surname
- Line 2 : First Name & Initials
- Line 3 :- Line 4 : USMC or USMCR and Gas mask size
- Line 5 : Religion preferredThe USAF is slightly different in that the blood type is on line 4 along with the gas mask size. For example:
